Due to the growth and opening of new territories we are hiring a Hospice RN! Puget Sound Home Health and Hospice is seeking an experienced and caring full-time Hospice Registered Nurse Case Manager (RN) in Southwest King County, Washington!
Benefits- $115,000 to $125,000 annually, DOE

- Perform patient assessments (initial + ongoing)
- Develop and update care plans
- Administer meds and manage symptoms
- Educate/support patients and families
- Attend/participate in IDT meetings
- Make home visits as needed
- Document in EMR accurately and timely
- Communicate with physicians and team
- Triage urgent calls/on-call rotation
- Guide on hospice philosophy/DNR/advance directives
- Provide end-of-life emotional support
- Mentor new staff
- Ensure compliance with regulations
- Coordinate with outside services (DME, pharmacy, etc.)
- Registered Nurse (RN) licensure
- Prior experience working in a hospice agency is strongly preferred
- Ability to work independently and demonstrate critical thinking
- Current BLS certification from AHA
- Licensed driver with an insured automobile in good working order
