Position Summary
Providence at Home with Compassus is seeking a compassionate and experienced Nurse Practitioner to provide hospice and palliative care across homes, long‑term care facilities, and hospitals. The NP will independently assess patients, coordinate care plans, and collaborate with the interdisciplinary team to deliver regulated, patient‑centered care while maintaining safety and quality standards.
Responsibilities- Conduct thorough medical, functional and psychosocial assessments and interpret diagnostic tests according to protocol.
- Develop, modify and implement individualized plans of care, including opioid therapy, IV medication, antibiotics, and feeding regimens as clinically indicated.
- Perform face‑to‑face, end‑of‑life and recertification assessment visits.
- Order medications and supplies in accordance with institutional protocols and maintain accurate documentation in the integrated patient record.
- Participate in interdisciplinary team meetings to recommend treatment plans and monitor ongoing care.
- Serve as the primary liaison and resource for patients, families and nursing staff regarding hospice and palliative care education.
- Assist patients and families with decisions regarding goals of care, withdrawal or withholding of interventions, and resuscitation status.
- Maintain schedules and productivity standards, ensuring timely completion of clinical tasks.
- Participate in quality improvement activities and safety initiatives to enhance patient care.
- Provide on‑call support in rotation with other providers as required.
- Assist in the referral of patients to appropriate health facilities and community resources.
• Master’s degree in Nursing.
• Minimum of 2 years’ experience in an acute or transitional care setting.
- Valid RN license in the state of practice.
- National Provider BLS certification (required upon hire).
The role requires regular standing, walking, and manual dexterity. The practitioner must be able to lift and move objects up to 50 pounds. Visual acuity, including close and distance vision, color perception and depth perception, is required. Exposure to bodily fluids and infectious diseases is expected. Routine use of standard medical equipment and tools associated with clinical care is essential.
