Hospice RN Case Manager

Puget Sound Home Health and Hospice is hiring a Hospice RN Case Manager in Pierce County, Washington. The role is full‑time with salary $105,000 – $115,000 annually (depending on experience).

• Complete comprehensive in‑home patient assessments
• Manage patient comfort, symptom control, and pain relief
• Develop and update individualized plans of care under physician orders
• Deliver skilled, compassionate nursing care in the home setting
• Document care accurately and promptly per clinical and legal standards
• Participate in quality improvement initiatives (QAPI)
• Collaborate with an interdisciplinary team to ensure holistic patient support

• Current RN license in WA (or a compact state)
• BLS certification from AHA
• Valid driver’s license, auto insurance, and reliable transportation
• Strong critical thinking and independent problem‑solving skills
• Hospice agency experience preferred but not required
