• Ensure appropriate reimbursement for patient care services.
• Participate in the Patient Assessment Process by completing items on the MDS and accompanying Care Area Assessments (CAAs).
• Assist in developing individualized patient care plans, identifying patient needs and projected outcomes as required by Federal and State regulations.
• Contribute to the interdisciplinary Resident Assessment Instrument (RAI) process, following RAI guidelines for completion, computer encoding, and electronic transmission of the MDS.
• Identify changes in patient condition in conjunction with the interdisciplinary team and coordinate scheduling and completion of indicated assessments.
• Assist with Activities of Daily Living training for new hires and provide ongoing continuing education to existing staff.
• Attend educational programs to maintain knowledge of current RAI/Prospective Payment System regulations.

• Must be a Registered Nurse currently licensed by the state and in good standing.
• Previous experience in a hospital, long-term facility, or other healthcare-related facility is required.
• Prior experience completing MDSs and Care Plans is required.
• Must be knowledgeable of general, rehabilitative, and restorative nursing and medical practices and procedures, as well as laws, regulations, and guidelines governing long-term care.

Restrictions apply based on collective bargaining agreements, applicable state law and factors such as pay classification, job grade, location, and length of service.

Posted Salary Range: USD $54.00 - $56.00 /Hr.
