In-Home Health Clinician - Nurse Practitioner or Physician Assistant Seattle​/King

Overview

Signify Health, part of CVS Health®, is seeking a Full‑time Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations. The clinician will visit members’ homes to perform a 45‑minute assessment that includes a medication and medical history review, a physical evaluation, and optional additional diagnostic tests such as diabetic eye exams or spirometry. The clinician will use an iPad and an intuitive clinical workflow and is not required to prescribe medication, order lab tests, or alter treatment regimens.

• Bring a compassionate, patient‑focused approach to every visit.
• Visit thousands of people nationwide, connecting them to appropriate care and services.
• Coordinate with Signify Health as a clinician colleague to fulfill the role requirements.
• Travel locally and within licensed states as needed.
• Conduct virtual visits when required.
• Provide all indicated Signify Health services, including diagnostic and preventive services such as spirometry and diabetic eye exams.
• Perform duties safely and comfortably in private homes that may contain pets, secondhand smoke, or other allergens.

• Active, unrestricted NP or PA license in the coverage area.
• Board certification (required); multi‑state licenses preferred.
• Experience in primary care or preventive health services.
• Ability to travel and work flexible hours, including evenings and weekends if necessary.
• Excellent communication and interpersonal skills.
