Inpatient Registered Nurse (RN) Care Manager
The Inpatient Registered Nurse (RN) Care Manager provides professional, comprehensive, patient‑centered care management services for at‑risk patients in an acute care environment. Accountabilities include assessment and planning, coordination of care, resource utilization management and/or review, discharge planning, documentation of interventions, regulatory compliance and patient advocacy. The goal is to ensure the use of appropriate healthcare resources throughout the continuum, so that the care provided is the right care, at the right time, in the right setting.
ResponsibilitiesAssess, plan, and coordinate care for at‑risk patients in an acute care setting.
Manage resource utilization and conduct reviews as required.
Develop and implement discharge plans and document interventions.
Ensure regulatory compliance and advocate for patients.
Required Qualifications- Associate’s Degree in Nursing upon hire
- Washington Registered Nurse License upon hire
- 2 years of acute care experience in a Medical Surgical or inpatient setting
- Bachelor’s Degree in Nursing or higher within 3 years of hire
- National certification in specialty area
- 1 year of experience in care management or utilization review or completion of TIP program or Case Management Orientation Program
