Overview

The Nurse Navigator is a leader providing support, mentorship, and guidance to those who provide direct patient care. The Nurse Navigator coordinates the delivery of patient care by assessing care requirements of the individual patient and makes recommendations based on severity of illness, intensity of service, and skill level. The Nurse Navigator ensures patients receive timely, safe, effective, and high-quality care.

They coordinate the patients plan of care with the bedside nurse and set expectations regarding professional conduct. The Nurse Navigator serves as a resource for the patient care unit and is responsible for accountability of direct and indirect care providers for professional responsibilities and adherence to regulatory guidelines. The Nurse Navigator manages the assigned inpatient and outpatient population to support an effective new care model that emphasizes quality patient care, safety, financial stewardship, and integration of a seamless care model, including discharge planning for post-acute services, LOS management, and utilization management.

Responsibilities
  • Coordinate and manage patient care from pre-op through recovery and follow-up
  • Provide patient and family education
  • Conduct inpatient rounding and assist with discharge planning
  • Collaborate with physicians, nurses, and support services
  • Monitor quality, patient outcomes, and length-of-stay metrics
  • Maintain accurate documentation in EPIC and utilization management systems
  • Serve as a clinical resource and mentor to staff
  • Support program growth, physician relationships, and community outreach

  • Shift: Days (No Nights, Weekends, or Holidays)
  • The Nurse Navigator serves as the primary guide for orthopedic surgery patients throughout their care journey and coordinates patient care, provides education and support, assists with discharge planning, and collaborates with physicians, nurses, and the healthcare team to ensure outcomes.
Qualifications
  • Required: Graduate of an accredited nursing program;
    Current RN license; BLS certification; 2 years of nursing experience
  • Preferred: BSN; ACLS certification;
    Orthopedic, care coordination, navigator, or case management experience
Physical Requirements
  • Lift, push, or pull up to 50 lbs
  • Prolonged standing and walking
  • Frequent bending, kneeling, reaching, twisting, and squatting
