Position Overview

As a Clinical Care Coordinator, you will oversee an assigned caseload of home care clients while serving as a clinical resource and leader for caregivers in the field. This position combines client assessments, supervisory visits, caregiver education, quality assurance, staffing collaboration, and direct client support to ensure exceptional client outcomes and continuity of care.

This is a full-time position based on a 40-hour work week. Approximately 75% of your time will be spent in the field conducting Start of Care visits, client assessments, supervisory visits, caregiver orientation, quality assurance visits, client introductions, and providing direct client support as needed. The remaining time is dedicated to care planning, documentation, compliance activities, provider communication, scheduling collaboration, and administrative responsibilities.

While every effort is made to maintain a consistent caseload and schedule, Clinical Care Coordinators are expected to remain flexible and adapt to changing client needs, hospitalizations, staffing demands, and agency operational priorities.

Schedule & Work Environment

This position is generally scheduled Monday through Friday during regular business hours, with flexibility based on client and operational needs.

Clinical Care Coordinators primarily work in residential and community-based settings, traveling between client locations while maintaining dedicated administrative time for documentation, care coordination, and interdisciplinary communication.
