General ResponsibilitiesAs part of a multidisciplinary team coordinate and deliver health care services for residents of Sunrise Services ESF. Responsible for administering/assisting residents with medications. Ensure that health records are complete and secure.
Essential Functions- Medication passes to a maximum of 16 residents.
- Documents medications in electronic medical records system.
- Makes basic behavioral observations, gathers resident clinical data and assists in identification of needs and problems relevant to the resident; and communicates clinical outcomes in a timely fashion to the supervisor.
- Identifies problems and assist in the development of nursing diagnoses which are central to the plan of care.
- Carries out the resident care plan performing common therapeutic techniques.
- Administers injectable medications to assigned residents.
- Collects bio-medical information from residents such as blood pressure, heart rate, weight, etc.
- Observes and documents residents for reported illness, accidents, and changes in physical, mental and emotional functioning.
- Encourages residents in health education, good nutrition, hydration and other activities related to good overall health and well being.
- Coordinates other aspects of resident care, sets appointments, medical specialists, etc.
- Keeps medical supply rooms stocked and maintains inventory.
- Serves as shift lead during assigned rotations.
- Education: AA degree in nursing or other required nursing education for an LPN.
- Experience: Two years of experience in providing health care and support to, menta lly ill, physically disabled , or developmentally disabled individuals. Any other acceptable combination of education and experience, which insures the ability to perform the duties of this position may be substituted for the above requirements.
- Licensure/Certification: Licensed Practical Nurse (LPN) per WAC 246-840.
- DSHS background clearance is required.
- Basic computer skills including Microsoft Office Word and Excel.
- Strong verbal and written communication skills, and demonstrated leadership ability.
- Demonstrated ability to exercise sound judgment in challenging dynamic situations including behavioral and staffing issues.
- Sit or stand for extended periods of time, perform light lifting (approximately 10-20 pounds), and physically assist unsteady residents.
- Must be able to perform physical aspects of position that may include carrying groceries, laundry, and other personal belongings; reaching at waist, above shoulders or below knees, walking up and down stairs, etc.
