Home Health LPN

• Build meaningful one-on-one relationships while providing skilled nursing care in patients' homes
• Assess patient needs, administer treatments, medications, and wound care services
• Educate and empower patients and families to manage their health with confidence
• Collaborate with physicians, therapists, and care team members to deliver outstanding care
• Document visits accurately and maintain compliance with clinical standards
• Help patients remain safe, independent, and comfortable in their own homes

• Current Washington LPN License
• Home Health or Hospice experience preferred, but not required
• Current BLS Certification
• Valid driver's license and insured vehicle

Demonstrates expertise in skilled nursing care, patient assessment, and treatment administration while fostering strong relationships with patients and families. Proficient in documentation and compliance with clinical standards in home health settings.
