Requirements

• Current state license as a Registered Nurse – Required
• Homecare Homebase Software Experience – Required
• OASIS Experience – Required
• 1-2 years experience as a Registered Nurse in a clinical care setting – Required
• 2+ years Home health experience – Required
• Proof of current CPR certification – Required
• Current driver's license – Required
• Must have reliable transportation and insurance – Required
• CHPN certification – Preferred
• Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program – Required
• Bachelors of Science in Nursing – Preferred

Responsibilities

• Assess home health and/or hospice patients to identify needs and develop a Plan of Care (POC).
• Implement and evaluate the Plan of Care to ensure quality and continuity of care.
• Educate patients and their families on care and promote self-care practices.
• Supervise and provide clinical directions to support staff to ensure quality service.
• Monitor assigned cases for compliance and perform on-call responsibilities as needed.
• Maintain effective communication with patients, families, and the interdisciplinary team.
