• Provide ongoing assessment of nursing care needs for a defined group of patients.
• Develop a written plan of care based on nursing diagnosis and patient needs.
• Assist patients and families in developing health care goals as part of the individualized plan of care.
• Provide nursing interventions safely and competently, including telephone consultations.
• Evaluate patients' responses to care and modify care plans in collaboration with the health care team.
• Assess and plan care for patients requiring hospitalization, initiating nursing admission database and discharge planning.
• Communicate patients' psychosocial, cultural, and emotional needs in nurse-patient interactions.
• Develop educational components for nursing care plans and provide teaching/counseling to patients and families.
• Integrate health promotion and preventive care into ongoing assessments.
• Participate in continued education programs and professional activities to enhance nursing practice.
