Young Adult Case Manager

Cocoon House seeks a Young Adult Case Manager to support youth experiencing or at risk of homelessness. In this role, you will provide trauma informed case management, crisis intervention, and advocacy to help clients achieve stability and sustainable independence. Responsibilities include conducting intakes and assessments, developing individualized service plans, coordinating housing, education, and employment resources, and facilitating life skills groups.

You will collaborate with community partners, maintain accurate documentation, ensure safety and confidentiality, and promote an inclusive, empowering, and social justice oriented environment.
