Youth Housing Case Manager

Cocoon House seeks a Youth Housing Case Manager to support at-risk and homeless youth in Snohomish County. In this role, you will manage a caseload of youth in shelter and transitional housing, conduct intakes and assessments, and develop individualized housing and stability plans. You will connect youth and families with community resources, coordinate services with partners, and advocate for their needs.

Working in a compassionate, trauma-informed, and inclusive team, you will maintain accurate documentation, support safety and crisis response, and help youth build skills for independence.
