Youth Housing Case Manager

Cocoon House seeks a Youth Housing Case Manager to support at-risk and homeless youth in Snohomish County. In this role, you will manage a caseload of youth in shelter and transitional housing, conduct intakes and assessments, and develop individualized housing and stability plans. You will connect youth and families with community resources, coordinate services with partners, and advocate for their needs.

Working in a compassionate, trauma-informed, and inclusive team, you will maintain accurate documentation, support safety and crisis response, and help youth build skills for independence.

Responsibilities

• Manage a caseload of youth in shelter and transitional housing programs
• Conduct intakes, psychosocial and housing stability assessments
• Develop and monitor individualized housing and service plans
• Provide trauma-informed case management and advocacy
• Coordinate referrals to community resources and benefits
• Collaborate with internal teams and external providers
• Support youth in building life, employment, and independent living skills
• Respond to crises and support safety planning
• Maintain timely, accurate case notes and data entry in databases
• Participate in case conferencing, trainings, and community outreach
