SUMMARY

This role provides support, resources, and guidance to clients, helping them navigate challenges related to housing, employment, mental health, and substance use. The Jail Transition Case Manager will work collaboratively with various stakeholders, including law enforcement, social services, and community organizations, to promote a smooth transition and reduce recidivism.

Pay

$27.65 - $29.70/ hour

ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Maintains a reliable attendance record and adheres to scheduling requirements based on business and clients’ needs.
  • Conducts comprehensive assessments of clients’ needs, including risk factors, strengths, and areas requiring support.
  • Develops, implements, and updates personalized transition plans that address housing, employment, education, and health care needs.
  • Connects clients with community resources such as housing assistance, job training programs, mental health services, and substance abuse treatment, and assists in obtaining necessary documentation for applicable benefits.
  • Maintains ongoing case management with regular follow-ups and progress monitoring, adjusting transition plans as necessary.
  • Provide appropriate crisis intervention and support to clients facing challenges during their transition period.
  • Advocates on behalf of clients to ensure they receive necessary services and support from various agencies and community organizations.
  • Facilitates groups or meetings with clients or partners on life skills, job readiness, and personal development.
  • Understands and practices safekeeping of client-protected health information per Bridgeways’ HIPAA policies and procedures, utilizing Bridgeways’ Electronic Medical Record system in a timely and accurate manner to ensure compliance.
  • Represents Bridgeways in community and regional activities, committees, and work groups as requested.

EDUCATION And/or EXPERIENCE
  • Bachelor’s degree in social work, psychology, criminal justice, or a related field (Master’s degree preferred) or equivalent experience.
  • Minimum two years’ experience working in a case management, social services, or criminal justice (criminal justice system preferred).
Mathematical Skills
  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
REASONING ABILITY
  • Ability to define problems, collect data, establish facts, and draw valid conclusions.
