Cocoon House seeks a Coordinated Entry Navigator to connect at-risk and homeless youth in Snohomish County to safe housing and critical resources. Based in Everett, this role conducts outreach, screenings, and assessments using trauma-informed, youth-centered practices. The Navigator coordinates referrals to shelters, transitional housing, and community services, maintains accurate data in HMIS or similar systems, and partners closely with schools, agencies, and families.

You'll join a compassionate, inclusive team dedicated to ending youth homelessness and empowering young people to build stable, independent futures.

Responsibilities

  • Conduct coordinated entry screenings and assessments for at-risk and homeless youth and families.
  • Provide navigation and referral to housing, shelter, and supportive services.
  • Engage in outreach in community settings, shelters, and schools.
  • Maintain accurate, timely client records in HMIS or similar databases.
  • Collaborate with internal programs and external partners to coordinate care.
  • Use trauma-informed, culturally responsive, and youth-centered approaches.
  • Educate youth and families on available resources and service options.
  • Track outcomes and support reporting and data quality efforts.
  • Participate in case consultations, team meetings, and community coordination efforts.
