Education & Case Manager

Cocoon House seeks an Education & Employment Case Manager to support at-risk and homeless youth in Snohomish County. In this role, you'll provide trauma-informed case management focused on education and job readiness, developing individualized service plans that connect youth to school, training, and employment opportunities. You'll coordinate with housing, outreach, and community partners, facilitate workshops on life and workforce skills, and track progress and outcomes.

Responsibilities

• Provide trauma-informed case management to at-risk and homeless youth focused on education and employment goals
• Develop and monitor individualized service and education/employment plans
• Connect youth with schools, training programs, employers, and community resources
• Coordinate services with housing, outreach, and internal Cocoon House programs
• Facilitate workshops on job readiness, life skills, and academic success
• Advocate for youth with community partners, employers, and educational institutions
• Maintain accurate, timely case notes, data, and outcome reports
• Conduct regular check-ins, crisis intervention, and safety planning as needed
• Support youth in removing barriers to school attendance and job retention
• Participate in team meetings, training, and continuous improvement efforts
