Legacy Healthcare LLC is seeking a dedicated Licensed Practical Nurse to join our team at Avantara Evergreen Park
, a distinguished skilled nursing and rehabilitation facility located at 10124 S Kedzie Ave, Evergreen Park, IL 60805.

Base pay range: $35.00/hr – $38.00/hr annual salary: $68,000 – $75,000 per year. Same day pay is available.

Qualifications:

• State of Illinois Licensed Practical Nurse (LPN) license, unencumbered.
• Prior LPN experience preferred; willingness to invest in the right candidate is welcomed.

Responsibilities:

• Ensure residents receive medication and nursing care as directed by their individual care plan.
• Conduct resident assessments.
• Act as a team leader and advocate for residents' well‑being and safety.
• Maintain thorough and accurate documentation.
• Collaborate with interdisciplinary team members to provide high‑quality care.
