You will assess patient needs, deliver skilled nursing care per the Plan of Care, and collaborate with an interdisciplinary team to refine individualized plans.
Requirements include an active RN license, CPR certification, reliable transportation, and at least 1 year of RN experience; hospice or palliative care preferred.
