Role DescriptionThis is a full-time, on-site Physical Therapist role based in Ewa Beach, HI. The Physical Therapist will provide direct patient care in outpatient and home-based settings, including comprehensive evaluations, development of individualized treatment plans, and delivery of evidence-based interventions. Daily responsibilities include monitoring patient progress, updating care plans, documenting services in accordance with regulatory and payer requirements, and communicating with referring physicians and other care team members.
The role also involves educating patients and caregivers on home exercise programs, safety, and strategies to maintain mobility and independence, while contributing to a supportive, patient-centered environment.
- Strong foundation in Physical Therapy and Rehabilitation, with the ability to design and adjust individualized treatment plans.
- Demonstrated skills in Patient Care and Home Care, including safe, effective hands-on treatment and patient education.
- Working knowledge of Medicine as it relates to musculoskeletal, neurological, and cardiopulmonary conditions commonly treated in outpatient and home-based settings.
- Current licensure or eligibility for licensure as a Physical Therapist in the state of Hawaii.
- Experience in outpatient or home health physical therapy preferred.
- Effective communication and interpersonal skills, with the ability to work collaboratively with patients, caregivers, and healthcare professionals.
- Strong organizational and time-management skills, including accurate documentation and adherence to clinical and insurance requirements.
