Job Summary

The Behavioral Health Nurse (LPN or RN) works in conjunction with behavioral health providers to provide professional, team‑based integrative care and nursing support in a clinical setting. Under clinical supervision of the Medical Director and administrative supervision of the Program Director, the nurse will play a critical role in the delivery of quality, patient‑centered integrative care within the context of a qualified partial care program.

Responsibilities
  • A. 1. Communicates with patients in a courteous and professional manner in face‑to‑face encounters and via telephone.
    2. Educates the patient and family member on the patient’s diagnosis and health care self‑management, with in scope of knowledge and authority.
  • B. 1. Participates with the Provider in the implementation of the plan of care and multidisciplinary patient education.
    2. Promotes the Team‑based Care Model by participating in daily huddles and departmental meetings to communicate and collaborate on patient issues and needs.
  • C. 1. Conducts initial nursing assessment for patients including: obtaining vital signs, reconciling medication lists and allergies, and identifying patient’s chief complaint.
    2. Assists in coordinating outside tests, procedures, and doctor’s appointments.
    3. Administers medications including IM and Sub Q injections ordered by Provider.
    4. Documents clinical activities and patient interactions in EMR, monitors utilization and reviews as needed.
    5. Assists provider in monitoring lab results, telephone encounters, and requests for refills. Communicates these outcomes and results with patients.
    6. Interacts with other agencies/medical entities regarding patient medical information and documents referrals when necessary.
    7. Other duties and responsibilities as assigned by the Medical Director and Program Director.
  • D. 1. Assists with ensuring that patient encounters are properly coded and documented.
  • E. 1. Demonstrates proficiency in management of Behavioral Health diagnosis, including knowledge of psychiatric medications (including side effects), substance abuse, and crisis intervention.
Qualifications
  • Bachelor’s degree in Nursing, with state licensure, required.
  • Experience in Case Management or Utilization Management desired.
  • Strong communication and interpersonal skills.
  • Strong analytical skills.
