Join Cornerstone VNA, an award-winning, mission-driven nonprofit dedicated to providing trusted, compassionate, expert care in the comfort of patients' homes. We're looking for a skilled and caring Registered Nurse (RN) to join our dedicated home care team serving the Rochester, NH community.

What You'll Do:

* Manage a diverse home care caseload with autonomy and flexibility

* Perform comprehensive nursing assessments and develop patient-centered plans of care

* Collaborate with physicians and interdisciplinary team members to coordinate high-quality care

* Monitor patient progress, adjust care plans, and educate patients and families on health management

* Complete OASIS assessments with accuracy

* Document care accurately and timely during patient visits

* Supervise Licensed Practical Nurses (LPNs) and Licensed Nursing Assistants (LNAs) as needed

* Participate in case conferences and discharge planning

* Follow the Nurse Practice Act, agency policies, and home health regulations

What You Bring:

* Current NH or Maine Registered Nurse (RN) license or other compact state license (or eligibility)

* Minimum 1 year of clinical nursing experience preferred (home care experience a plus)

* Excellent clinical assessment, communication, and organizational skills

* Ability to work independently while being part of a collaborative care team

* Comfort working in a variety of home environments

* Reliable transportation and valid driver's license (Mileage reimbursed!)
