LPN needed for a Long Term Care facility in Exeter, New Hampshire. You will provide direct resident care in a LTC setting, including medication administration, treatments, wound care, monitoring for changes in condition, and clear communication with the RN and provider team. Expect a steady pace with strong teamwork, consistent documentation, and a focus on resident comfort, safety, and dignity. Candidates should be comfortable supporting a primarily geriatric population and keeping calm during busy med passes and change of shift.

Active LPN license required (NH) BLS preferred (facility dependent) Recent LTC or SNF experience preferred Comfortable with med passes, injections, tube feeds, wound care, vitals, and basic assessments within LPN scope EMR charting and accurate documentation required Floating within the facility may be required based on census Must be able to pass background, drug screen, and complete all facility compliance items on time Flexibility with scheduling, including weekends and holidays as needed
