River Woods Exeter is seeking a Part Time RN or LPN to provide direct resident care in a team‑focused environment where residents are at the heart of what we do.
$3000 Sign on Bonus

- Perform professional duties in accordance with facility policies, procedures, standards of practice, the State of New Hampshire Nurse Practice Act, OBRA, and HIPAA.
- Collaborate with the interdisciplinary team to formulate and update individualized resident care plans; participate in resident care plan meetings.
- Institute care protocols including falls prevention, safety and elopement, behavior management, weight management, skin integrity, bowel/bladder management, infection control guidelines (standard precautions), and pain assessment and management.
- Assess and document changes in residents’ physical or mental status and report findings to the primary care physician and treatment team; provide complete clinical data to facilitate decision‑making.
- Provide leadership and direction for direct caregivers; prepare daily assignments and delegate workload appropriately; supervise direct care through observation and evaluation of results.
- Establish positive working relationships with team members, residents, and family members to facilitate open, direct communication.
- Obtain, administer, and document medications and treatments given to residents following professional guidelines and nursing department policies; inventory and maintain locked system for narcotics.
- Support, create, and maintain a positive culture for team members and residents.
- Must possess, as a minimum, a nursing degree or diploma from an accredited college, university, or nursing school.
- Long‑term care facility experience and specialized knowledge of nursing care of the elderly and rehabilitative nursing is preferred.
- Must hold a registered nursing (RN or LPN) license in the State of New Hampshire. Certification in intravenous administration is preferred. Must maintain CEUs annually. CPR is required.
