We're looking for a skilled and caring Registered Nurse (RN) to join our dedicated home care team serving the Rochester, NH community. If you are passionate about autonomy, flexibility, and meaningful one-on-one patient care, this is the role for you!
What You’ll Do:- Manage a diverse home care caseload with autonomy and flexibility
- Perform comprehensive nursing assessments and develop patient-centered plans of care
- Collaborate with physicians and interdisciplinary team members to coordinate high-quality care
- Monitor patient progress, adjust care plans, and educate patients and families on health management
- Complete OASIS assessments with accuracy
- Document care accurately and timely during patient visits
- Supervise Licensed Practical Nurses (LPNs) and Licensed Nursing Assistants (LNAs) as needed
- Participate in case conferences and discharge planning
- Follow the Nurse Practice Act, agency policies, and home health regulations
- Current NH or Maine Registered Nurse (RN) license or other compact state license(or eligibility)
- Minimum 1 year of clinical nursing experience preferred (home care experience a plus)
- Excellent clinical assessment, communication, and organizational skills
- Ability to work independently while being part of a collaborative care team
- Comfort working in a variety of home environments
- Reliable transportation and valid driver's license (Mileage reimbursed!)
