Licensed Practical Nurse

Responsibilities

• Provide direct nursing care to residents in independent living, assisted living, and nursing care settings
• Administer medications and treatments according to physician orders and facility protocols
• Monitor, document, and report changes in resident condition to the interdisciplinary team
• Develop, implement, and update individualized care plans
• Educate residents and families on health conditions, medications, and wellness practices
• Collaborate with CNAs and other care team members to ensure high-quality, person-centered care
• Maintain accurate clinical records and comply with all regulatory and safety standards
• Support a compassionate, respectful community-focused environment
