Position: In-home Intercept Case Manager - Region 6

Travel Requirement

Clients are serviced in Bergen, Essex, and Hudson counties. Travel to client sites is required.

Work Schedule

Full-time, 40 hours per week; hybrid work arrangement.

Compensation

Base salary: $52,500 + $2,000 on-call.
Bilingual salary: $54,500 + $2,000 on-call.

Job Summary

As an In-home Intercept Case Manager, you will provide individualized family sessions in the home and community settings to keep youth safe in the home. Working with approximately five families, you will conduct an average of three face‑to‑face sessions per week to divert youth from out‑of‑home placements and strengthen the family unit.

Responsibilities
  • Provide individualized family sessions in the home and community.
  • Manage a caseload of five families.
  • Conduct an average of three face‑to‑face sessions per week to support and strengthen the family unit.
  • Attend three required meetings per week (group supervision, clinical consultation, and individual development).
  • Complete all documentation related to families, including bi‑weekly treatment plans, weekly session notes, and ongoing clinical assessments.
  • Advocate for families within related systems to gather information for intervention development.
  • Collaborate with DCP&P staff to assist families in addressing needs and strengthening the family system.
  • Provide after‑hours support to families on a rotating on‑call schedule and respond in‑person to some crisis situations as necessary.
Qualifications
  • Master's degree in social work, psychology, or a related field, or
  • Bachelor's degree in social services with one year of related experience in counseling or case management.
  • A valid driver’s license that is insurable by the agency’s automobile insurance.
