This position provides comprehensive care coordination for patients as assigned. The role assesses each patient's plan of care and develops, implements, monitors, and documents the utilization of resources, ensuring the patient's progress through their care and facilitating options and services that meet the patient's health care needs. The intensity of care coordination is situational and appropriate based on patient need and payer requirements.
The position is accountable for the quality of clinical services delivered by both the coordinator and others, identifying and resolving barriers that may hinder effective patient care.
Fairbanks Memorial Hospital is a non‑profit facility owned by the Greater Fairbanks Community Hospital Foundation and a Joint Commission‑accredited facility with 152 licensed beds. Responsibilities- Manages individual patients across the health care continuum to achieve optimal clinical, financial, operational, and satisfaction outcomes.
- Acts in a leadership role with process improvement activities for populations of patients to achieve optimal outcomes.
- Collaboratively develops and manages the interdisciplinary patient discharge plan, communicating it effectively across the continuum of care.
- Evaluates medical necessity and appropriateness of care, optimizing patient outcomes; assesses admissions and continued stay using standard criteria; identifies issues that may delay discharge and facilitates resolution.
- Establishes and promotes a collaborative relationship with physicians, payers, and other health care team members, collecting and communicating pertinent, timely information to fulfill utilization and regulatory requirements.
- Educates internal health care team members on case management and managed care concepts, facilitating integration into daily practice.
- May supervise other staff.
Performs all functions according to established policies, procedures, regulatory and accreditation requirements, as well as applicable professional standards. Provides all customers of Foundation Health with an excellent service experience by consistently demonstrating core and leader behaviors each and every day.
Must possess knowledge of case management or utilization review normally obtained through completion of a bachelor’s degree in case management or health care. Requires a current Registered Nurse (R.N.) license in the state where the assignment is located. For assignments in an acute care setting, Basic Life Support (BLS) certification is also required. Requires a proficiency level typically achieved with 5 years clinical experience.
Must have a working knowledge of care management, acute care and/or home care environments, community resources and resource/utilization management, and demonstrate critical thinking, problem‑solving, effective communication, and time management skills. Must be able to work effectively in an interdisciplinary team. For acute care assignments, must work flexible hours and take rotating after‑hours call. Foundation Registry and Travel positions require a minimum of one year experience in an acute care hospital and/or home care setting;
experience must include working in an acute care and/or home care setting within the past 12 months as a Case Manager in the specialty area.
CCM (Certified Case Manager) preferred.
