Overview
Home Sweet Home Health - Fairbanks Memorial Hospital is looking for a full-time Registered Nurse (RN) to join the Home Health Care team in Alaska's "Golden Heart City". Fairbanks, Alaska is located in the interior of the state where you can find adventure, culture, solitude or scenery. Home Health nurses provide care and assist with coordination of needs to help patients meet their full potential.
Must possess a current RN license within the State of Alaska. BLS certification required.
- Evaluate, plan, administer, and supervise nursing treatment in accordance with physician referral, state and national organizational standards, regulatory agencies, and established departmental policies and procedures. Provide competent care appropriate to the age of patients served in a Home Health Care setting.
- Assess patient physical, psychological, social, spiritual, educational, developmental, cultural, and discharge planning needs. Review patient history and physical with patient/family, analyze diagnostics, and report results as needed. Complete assessments and reassessments per policy.
- Formulate and implement a plan of care, including discharge planning, with input from patient, family, and health team. Set realistic, measurable goals in collaboration with the patient/family; adjust plans as patient needs change.
- Coordinate and supervise care, delegate appropriately, and communicate patient status to the care team to ensure continuity of care and compliance with policies and standards.
- Evaluate patient progress and teaching effectiveness; revise plans as needed in collaboration with the health care team.
- Document assessment, planning, implementation, and evaluation in the patient record in a timely and accurate manner, reflecting objective/subjective data, nursing interventions, and patient responses.
- Promote evidence-based care and participate in professional development, unit/shared leadership, and precepting or mentoring where appropriate.
- Contribute to safe, timely, and equitable patient outcomes and participate in efforts to improve nursing quality indicators and outcomes.
- In some roles, supervise staff and workflow of the department.
Required: Must possess a valid current Alaska RN license. BLS certification required. Knowledge and skills typically demonstrated by an Associate's degree in nursing. May require ACLS, PALS, or additional certifications based on area of practice. Strong communication and problem-solving skills.
Preferred: Bachelor’s degree; professional certification; experience in an acute care setting within the past 12 months in the specialty area.
Pay & Benefits- Compensation: $38.42-$63.35 hourly, based on experience and education
- Retention Bonus: $10,000 - $15,000 (DOE)
- Relocation Assistance up to $7,500
- Temporary Housing: 90 days provided
