Organization OverviewThe Fairbanks Rescue Mission is a faith-based nonprofit organization dedicated to serving individuals and families experiencing homelessness with compassion, integrity, and professionalism. Through shelter services, outreach, and supportive programs, the Mission provides hope, stability, and practical assistance to those in need throughout Alaska.
Position SummaryThe SSVF Case Manager provides direct case management services to eligible Veterans and Veteran families in the Juneau area who are experiencing homelessness or are at risk of becoming homeless. The Case Manager works collaboratively with the Fairbanks Rescue Mission team and community partners to help participants obtain stable housing, access benefits, and achieve long-term self-sufficiency.
Key Responsibilities- Provide individual case management services to program participants at scheduled intervals.
- Develop individualized housing stabilization plans with Veterans and their families.
- Document all client interactions, services, and outcomes accurately and in a timely manner.
- Assist clients in obtaining and maintaining stable housing.
- Provide or coordinate supportive services including housing referrals, VA benefits navigation, employment services, budgeting assistance, and referrals for medical, mental health, and community services.
- Conduct outreach to community partners, property owners, agencies, and unsheltered Veterans.
- Assist with program intake, assessments, discharge planning, and aftercare support.
- Maintain accurate case files and program documentation in accordance with program requirements.
- Provide regular updates to the SSVF Program Manager and work collaboratively with staff and community partners.
- Demonstrate professionalism, compassion, and respect when serving Veterans and their families.
- Associate’s Degree in Human Services, Social Work, or a related field preferred, or equivalent experience.
- Minimum 2 years of case management or related experience preferred.
- Experience working with homeless or at-risk Veterans and families strongly preferred.
- Strong verbal, written, and interpersonal communication skills.
- Ability to work independently and as part of a team while maintaining strict confidentiality.
- Strong organizational and problem-solving skills.
- Proficiency in Microsoft Office and general computer literacy.
- Valid Alaska Driver’s License required.
- Ability to sit, bend, and lift up to 40 pounds as needed.
