Physical Therapist (PT)

Full-Time | Part-Time | PRN

$10,000 Sign-On Bonus for Full-Time PositionsH-1B Visa Sponsorship Available

We are seeking a licensed Physical Therapist (PT) to join our team and provide high-quality, patient-centered therapy services to residents in a supportive long-term care environment.

This is an excellent opportunity for a therapist who is passionate about helping patients improve their mobility, function, independence, and overall quality of life while working collaboratively with an interdisciplinary care team.

Key Responsibilities
  • Evaluate residents and develop individualized treatment plans based on clinical needs and physician orders.

  • Provide skilled physical therapy services in accordance with established care plans.

  • Complete accurate and timely documentation, including evaluations, treatment notes, progress reports, and discharge summaries.

  • Collaborate with nursing, therapy, and interdisciplinary team members to support resident outcomes.

  • Participate in care planning, utilization review, and interdisciplinary meetings as needed.

  • Educate residents, families, and staff regarding therapy goals, treatment plans, safety, and functional mobility.

  • Maintain established productivity, documentation, and quality standards.

  • Support regulatory compliance and survey readiness.

  • Promote resident safety, dignity, independence, and quality of life.

Qualifications
  • Graduate of an accredited Physical Therapy program.

  • Current and active state license as a Physical Therapist.

  • Long-term care or skilled nursing experience preferred.

  • Strong clinical documentation and communication skills.

  • Ability to work effectively as part of a multidisciplinary team.

  • Patient-focused, compassionate, and committed to providing quality care.

Physical Requirements
  • Ability to stand, walk, bend, kneel, and assist residents during therapy activities.

  • Ability to safely lift, transfer, and support residents as required.

  • Ability to work in a clinical and long-term care environment.
