The Medical Social Worker I (MSW) provides patients with access to community resources and ensures the patient’s medical, emotional, and safety needs are met. The MSW I follows all legal rules and regulations. The Company adopts the social work scope of practice as outlined in the Administrative rules of the state for its licensed MSW personnel. The MSW I is responsible for any and all practices and duties within the scope of practice as outlined by the state.###

Responsibilities
* Conducts assessment of patient needs and identifies any barriers
* Participates in the development of the plan of care
* Observes and monitors patient conditions
* Informs the physician, nursing supervisor, and other essential staff of changes in the client’s condition when necessary
* Educates team members in understanding significant social and emotional factors related to the health problems of the patient and acts as a consultant to other personnel
* Assists patient and family to understand, accept, and follow medical recommendations and how to utilize home health care services
* Connects patients and family to community resources and support
* Assists with patient financial aid applications
* Assists in long range planning
* Counsels the patient and family related to coping, changes, and grief
