Caregivers Home Health Services is looking to add multiple Hospice RNs to their growing team of clinicians.

- Coordinates hospice care by working with other team members as well as with the patients/family members to ensureneeds are met.
- Evaluate the effectiveness ofhospice services to the patient and family on an ongoing basis.
- Perform evaluations for hospice carealong with informational visits as needed.
- Utilize the nursing process toprovide professional nursing care.
- Provide guidance and supervision tothe LPN and supervises the LPN a minimum of every 30 days.
- Prepare the care plan for the Hospice Aide/Home Attendant.
- Must be a graduate from an accredited School of Nursing
- Must be licensed in the State of Virginia as a Registered Nurse
- Hospice/Home care experience preferred (extensive training provided)
