Schedule:
M‑F 8:30am– 5:00pm with on‑call requirements of 1 weekend per month, 2 evenings per month, 1 summer holiday and 1 winter holiday. Territory:
Fairfax, Falls Church, McLean areas.
Registered Nurse Case Manager (RNCM) assumes ultimate accountability and leadership for assessment of patient condition and plan of care. The RNCM provides case management, clinical care and is responsible for coordination of all disciplines involved in providing quality, cost‑effective and billable care through direct care and supervision of care under the direction of the physician. The RN educates patients and their families and ensures patient safety, accurate documentation and compliance with all legal rules and regulations.

- Registered Nurse with current license in the state of employment.
- Minimum of one year of experience as an RN in an acute care setting. Home‑care experience desired.
- Effective communication skills and ability to create positive impressions with patients, families, physicians and co‑workers.
- Calm, patient, and accommodating with a compassionate and caring attitude.
- Knowledgeable in nursing best practices and able to make appropriate nursing judgments.
- Detail‑oriented, observant, disciplined work ethic, and ability to prioritize and be timely.
- Ability to follow directions and work as a team member.
- Valid driver’s license and auto insurance with your name as a listed driver.
- Oversee the overall care of the patient from beginning to end of episode.
- Update all doctor orders in patient chart accordingly.
- Notify doctor of any significant changes in patient’s condition.
- Manage multi‑disciplinary care as applicable.
- Provide skilled nursing care in patient’s home as ordered by the attending physician.
- Observe and monitor patient conditions.
- Perform OASIS assessments to develop an individualized plan of care and make adjustments as needs change.
- Administer medication as prescribed by the physician.
- Help decrease re‑hospitalizations by front‑loading visits for high‑risk patients.
- Promote continuity of care with appropriate admissions, transfers and discharges.
- Counsel patient and family on disease/injury processes and how to manage.
- Incorporate patient and family in development of plan of care.
- Teach patients on preventative measures and good decision making.
- Listen to patient and family members.
- Oversee and supervise total care of patient provided by nurse aides and LPNs.
- Evaluate treatments and medical condition of the patient according to the plan of care and determine if the level of care meets the patients’ needs.
- Communicate with LPNs and nurse aides on supervisory visit results.
- Complete all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines.
- Maintain active RN license and communicate with scheduler for changes outside the normal schedule.
- Participate in all on‑call requirements and case conferences, in‑service trainings and mandatory agency meetings.
- Drive to patient’s primary location per scheduled visit.
- Attend daily at assigned visit locations.
- Document all aspects of subsequent, discharge, eval/recert/resumption of care visits within 24hours of visits.
- Document all aspects of start of care visits within 48hours of visits.
