• Provide oversight of coordination of all clinical disciplines to deliver quality, cost‑effective billable care.
• Maintain relationships with partners to achieve clinical excellence, regulatory compliance, and patient and employee satisfaction.
• Directly supervise, lead, and develop the Director of Nursing and guide field staff and clinical support team.
• Monitor patient trends and outcomes, manage budget and key performance indicators such as admissions, recertifications, HHRG.
• Communicate with internal teams, patients, families, and physicians via phone, email, and face‑to‑face.
• Ensure patient and employee satisfaction and resolution of issues.
• Maintain accountability for policies, procedures, documentation, and charting practices in accordance with agency protocol and federal/state guidelines.
• Deliver results for budget and collaborate with health partners and care integration team on quality and process measures.

• Bachelor’s degree, RN, PT, or equivalent per state requirements.
• Registered Nurse in the state of employment preferred.
• Minimum five years supervisory or administrative experience in home healthcare or related programs.
• Expertise and home‑care background for multiple sites preferred.
• Knowledge of guidelines governing home health agencies.
• Knowledge of nursing best practices.
• Operational knowledge of HCHB software, PDAs, ADP, Microsoft Office, and Outlook preferred.
• Valid driver’s license and auto insurance.
• Capable of meeting all physical demands.
