Job Description

Inova Fair Oaks Hospital is looking for a dedicated Registered Nurse Case Manager to join the Case Management Team. This role will be Part‑Time Day shift, 20 hours per week, from 8:00 AM to 4:30 PM. Inova Fair Oaks is a top‑ranked 174 bed acute care community hospital serving the rapidly growing suburbs of Northern Virginia. The RN Case Manager I provides discharge planning and continuity of care for assigned patients in acute and post‑acute settings, coordinates services, acts as key liaison among patients, families, and interdisciplinary healthcare members, and utilizes utilization management techniques.

Job Responsibilities
  • Collects data for performance and outcome indicators; assists in reporting resource and financial indicators such as case mix, LOS, cost per case, excess days, readmission rates, denials, and appeals; analyzes variances to drive practice changes.
  • Uses pathway data with other disciplines to ensure effective patient management and adheres to policies, procedures, and standards while maintaining productivity and accuracy.
  • Provides discharge planning and continuity of care for assigned patients in acute and post‑acute settings; initiates and facilitates referrals to clinics, home health, hospice, SNF, acute rehab, LTAC, TCM, medical equipment, and supplies as indicated.
  • Collaborates with the interdisciplinary healthcare team, patients, and families to assess and coordinate discharge planning needs and transition of care to post‑discharge settings.
  • Provides educational resources or referrals for patients’ social or financial needs and advocates for patients to resolve barriers to care progression.
  • Discusses payer criteria with clinical staff, follows up to resolve issues with payers, and applies approved clinical criteria to monitor appropriateness of admissions and post‑acute stays.
  • Identifies at‑risk populations using screening tools, monitors LOS and resource use, and takes actions to achieve continuous improvement efficiencies.
Minimum Qualifications
  • Certification:
    Basic Life Support (American Heart Association).
  • Licensure:
    Licensed or eligible for licensure in Virginia as a Registered Nurse or an active multi‑state RN license.
  • Experience:

    1 year of case management and/or clinical care experience.
  • Education:

    Bachelor’s Degree in Nursing or Associate’s Degree. If holding an Associate’s Degree, must have a written BSN completion plan within 2 years of hire and complete the BSN within 5 years.
Preferred Qualifications
  • One year of previous inpatient case management experience and discharge planning.
