Transplant Coordinator – Post Heart

Inova Fairfax Hospital is looking for a dedicated Transplant Coordinator – Post Heart to join the team. This role is full‑time, with regular office hours Monday‑Friday and an on‑call requirement of approximately 3 weekends per year, and 2 weekday nights per month for remote triage. Sign‑on bonus and relocation available.

• Assess patient needs and collaborate with the healthcare team to address actual and potential patient‑care concerns.
• Complete documentation of plans, orders, and outcomes related to the pathway according to policy.
• Delegate patient‑care activities appropriately and manage communication for assigned caseloads.
• Educate patients, families, and providers; plan, implement, and evaluate formal and informal education regarding all aspects of care, documenting appropriately.
• Organize and submit data to ensure compliance with regulatory guidelines; maintain entry, review, and validation of all required data for the caseload, achieving >95% compliance.
• Actively participate in program quality assessment and performance‑improvement initiatives, including committees or teams.
• Assist with process and protocol development/revision based on evidence‑based best practice findings.
• Collaborate effectively with the interdisciplinary team to achieve patient outcomes.
• Continuously improve practice through continuing education and maintaining professional certification.
• Perform other duties as assigned.

• Education
  :
  Bachelor’s degree in nursing.
• Experience
  :
  Three years of RN experience.
• Certification
  :
  Licensed in the Commonwealth of Virginia as a Registered Nurse;
  Basic Life Support from the American Heart Association.

• Experience
  : critical care/med‑surg.
