Position: Registered Nurse (RN)

The Registered Nurse Case Manager (RNCM) assumes ultimate accountability and leadership for the assessment of the patient condition and plan of care. The RNCM provides case management, clinical care and is responsible for coordination of all disciplines involved in providing quality, cost‑effective and billable care through direct care and supervision of care under the direction of the physician. The RN educates patients and their family members and ensures the safety of the patient.

The RN is accountable for completing accurate documentation and remains compliant with all legal rules and regulations. The RN is responsible for all practices and duties within the scope of practice as outlined by the state.

Qualifications & Attributes
  • Registered Nurse with a current license in the state of employment
  • Minimum of one year of experience as an RN in an acute care setting
  • Home care experience desired
  • Effective communicator with patients, families, physicians and co‑workers
  • Calm, patient and accommodating in challenging situations
  • Compassionate and caring while working with patients
  • Knowledgeable of nursing best practices and able to make appropriate judgments
  • Detail‑oriented and observant, with the ability to prioritize and act in a timely manner
  • Dependent on direction and able to work as a team member
  • Valid driver’s license and auto insurance with the employee listed as a driver
Major Areas of Responsibility
  • Oversee overall care of the patient from beginning to end of the episode
  • Update all doctor orders in patient chart and notify doctor of any significant changes
  • Manage multidisciplinary care and provide skilled nursing care in the patient’s home as ordered
  • Observe and monitor patient conditions, perform OASIS assessments and adjust care plans as needed
  • Administer medication as prescribed and help reduce re‑hospitalizations through proactive visits and education
  • Promote continuity of care with appropriate admissions, transfers and discharges
  • Counsel patient and family on disease/injury processes and preventive measures
  • Incorporate patient and family input into care plan development
  • Supervise nurse aides and LPNs, evaluating treatments and ensuring care meets patient needs
  • Maintain active RN license and complete all required clinical documentation in accordance with agency protocol and federal guidelines
  • Participate in on‑call requirements, case conferences, in‑service trainings and agency meetings
Essential Functions
  • Drive to the patient’s primary location per scheduled visit and attend all assigned visit locations daily
  • Document all aspects of care—discharge, evaluation, re‑certification, or resumption—within 24 hours of visits and start‑of‑care visits within 48 hours
  • Submit all required documentation within specified company requirements and follow plan of care within scope of practice
  • Fulfill RN visits in assigned geographic location per patient need within federal and state specifications
Health Qualifications

Must be able to meet all physical position requirements, including a negative tuberculin skin test or chest X‑ray at orientation and annually thereafter, and be free of physical or medical conditions that would limit or restrict the ability to perform job functions. The role requires travel, the ability to drive, and readiness to work in a patient‑care environment with potential exposure to blood borne pathogens.
