Heartland Hospice in Virginia seeks an experienced Hospice Clinical Liaison to manage intake, referrals, and admissions, building strong relationships with hospitals, physicians, case managers, and discharge planners.
The role requires an RN license with 3–5 years hospice experience; a BSN is preferred. You will visit hospitals and facilities, educate families about hospice, and ensure compliant documentation while coordinating care. Friday–Monday, 10-hour shifts; excellent benefits.
