About the RoleThe case manager works under the supervision of the Senior Case Manager and assists low‑income families who have experienced homelessness or are er the Family PASS Self‑Sufficiency Program, the manager delivers long‑term intensive case management for up to three years, collaborates with county resources, and bentuk supports clients in attaining education and skills training to achieve economic self‑sufficiency. The caseload includes 15–20 households, and the manager applies a trauma‑informed, strengths‑based, and client‑centered approach.
Collaborative teamwork with supervisors helps achieve program goals aligned with agency philosophy and best practices for homelessness and domestic violence survivors.
- Interview and screen clients for eligibility for Family PASS’s Self‑Sufficiency Program.
- Assess client strengths and needs, completing ongoing biopsychosocial‑spiritual assessments to determine stability.
- Develop and revise individualized client‑centered case plans with goals and action steps.
- Coordinate linkage and referral to community resources including education/training, career counseling, mental health services, financial literacy, legal services, childcare, medical/dental services, etc.
- Employ a holistic approach, consulting with supervisors and other programs to help families overcome barriers.
- Advocate for client needs to ensure housing and financial stability.
- Plan, schedule, coordinate, and facilitate client meetings as needed.
- Participate actively in staff meetings, training, supervision, and other required gatherings.
- Create, track, and maintain client case files following agency and best practice standards.
- Document client contacts and case management activities in relevant databases.
- Input client data into lists and databases accurately and timely.
- Perform additional duties as necessary to support program and agency operations.
- Bachelor’s degree in social work, human services, related field, or comparable experience.
- 1+ year of case management or direct services.
- Strong written and oral communication skills.
- Ability to prioritize competing priorities and make sound judgments.
- Ability to complete tasks while navigating frequent interruptions.
- Creative, resourceful problem‑solving skills.
- Strong organizational and time‑management skills.
- Valid driver’s license, reliable personal transportation, up to 50% local travel required.
- Capability to drive a 15‑passenger van or minivan.
- Foreign language fluency other than English (e.g., Spanish, Farsi, Arabic, Amharic).
- Experience providing services to clients dealing with domestic violence, homelessness, mental health, or substance abuse issues.
- Annual TB Test required yea.
- Ability to sit or stand for long periods.
- Ability to lift items weighing 10–20 pounds.
