We are seeking a Bilingual Spanish speaking Social Worker / Case Manager to provide community-based case management and behavioral health support to youth and families in Northern Virginia. In this role, you will support youth receiving services through the Children's Services Act (CSA), which provides service planning for children with significant emotional or behavioral challenges and their families in Virginia.
This position focuses on Spanish speaking and immigrant youth and families and serves as the Lead Case Manager coordinating care for the youth and their family.

This position has a salary range in the low to mid 60s and is an on-site role.
In this role, you will be responsible for:
Case Management and Family Support
* Conduct assessments, including the Child and Adolescent Needs and Strengths (CANS), and gather relevant history for the Family Assessment Planning Team
* Assist youth and families in accessing behavioral health services and community supports
* Develop Individualized Family Service Plans / Mutual Assessment Plans including natural supports and transition planning
* Serve as a liaison between families and service providers
Collaboration and Service Coordination
* Participate in Family Resource and Family Planning Meetings and collaborate with providers to ensure coordinated care
* Monitor service quality and client progress through regular contact with families, providers, and team members
Documentation and Compliance
* Maintain documentation in the agency electronic health record system and ensure compliance with CSA policies, state contracts, and NVFS guidelines
* Maintain client files, follow confidentiality and mandated reporting requirements, and complete responsibilities assigned to CSA case managers and TBP participants
Program Participation
* Prepare required reports, participate in supervision, meetings, and trainings, and support program development, outreach, and agency initiatives as needed.
Your Background:
We understand that you may obtain skills through a variety of professional, personal, educational, and volunteer experiences.
* Master's degree in Social Work, Marriage and Family Therapy, Counseling or equivalent
* Oral and written fluency in English;
Spanish language fluency required
* Must have at least two (2) years of related direct service experience providing:
* individual and/or group services to youth and families,
* case management, and
* working with youth with behavioral health care needs
* Experience providing services to Spanish speaking immigrants and recently reunified families and youth strongly preferred
* Digital and technical proficiency including working knowledge of Microsoft Office Suite
* Must be able to demonstrate and live the organizational values of Communication, Integrity, Respect, Collaboration, and Accountability.
Additional Requirements:
* Successful completion of background checks upon hire
* Must clear Motor Vehicle Record check, have valid S. driver's license, and have own transportation in order to transport clients
* Must have reliable transportation to visit client homes and other designated community locations
* May be called upon to work beyond normal working hours
