The Contractor will provide professional foster care social work and case management services to children and youth placed in foster care within the Fairfax County/Falls Church service area. The Contractor will work in coordination with the Department of Family Services (DSS), foster families, children and youth, biological families when applicable, and other authorized service providers to support the safety, stability, well-being, permanency, and successful development of assigned children and youth.
The Contractor will provide services consistent with applicable federal, state, and local laws and regulations, Virginia child welfare requirements, DSS policies and procedures, and the terms and conditions of the applicable contract. The Contractor will maintain professional standards, protect confidential information, and ensure timely completion of all required case management activities, documentation, and contractual deliverables.
Essential Functions
- Provide foster care case management services to children, youth, and foster families in coordination with Fairfax County/Falls Church DSS.
- Conduct required face-to-face and/or virtual visits and maintain ongoing case contact.
- Assess client needs and develop, monitor, and update service and permanency plans.
- Coordinate placement, medical, behavioral health, educational, legal, and community-based services.
- Participate in placement planning, case conferences, team meetings, court-related activities, and permanency planning.
- Support foster families with case-related concerns, resources, referrals, and behavior management.
- Maintain communication with DSS, biological families when appropriate, and other service providers.
- Complete timely and accurate case records, reports, service plans, and other contractual documentation.
- Monitor placement stability, report safety concerns, and maintain compliance with DSS and contractual requirements.
- Maintain confidentiality and participate in required training, quality assurance, and on-call coverage.
- Travel within the designated service area and perform other duties within the scope of the contract.
- Bachelor's or Master's degree in Social Work or a related human services field.
- At least one year of experience providing case management or social services to children, youth, or families.
- Child welfare or foster care experience preferred.
- Knowledge of trauma-informed and culturally responsive practices.
- Strong written and verbal communication skills.
- Ability to complete comprehensive and timely case documentation.
- Successful completion of required background checks and clearances.
- Valid driver's license, insurance, and reliable transportation required.
- Spanish/English bilingual preferred.
