HHS require being in the field visiting patients in their homes. This is a fully in-person, home health role. Remote or hybrid work is not available.

Solano County:
Vallejo, American Canyon, Hercules and minimal visits to Napa County.

Speech Therapist/Language Pathologist (ST) – Home Health

• In partnership with the Case Manager, provide skilled services to assist patients with breathing, speaking, and swallowing functions.
• Evaluate and treat speech, language, cognitive and fluency disorders that affect communication.
• Serve as an educator, patient advocate, and member of the interdisciplinary care team.

• Current unrestricted CA Speech Language Pathologist license
• Current BLS/CPR certification
• Valid CA driver’s license
• Knowledge of development, behavior, and the influence of culture on health care
• Knowledge of state and federal homecare regulatory guidelines (preferred)
• Home health care experience is highly preferred
• Experience with high-acuity patients is a plus
• Experience working with an interdisciplinary team is highly desirable
• Case Management fundamentals is highly desired
• Self-directed, empathetic, critical thinker with outstanding interpersonal skills
• Leadership skills and organized
• Prior to scheduling an in-person interview, completion of a computer literacy and competency assessment is required to ensure you can navigate our environment, including electronic charting and our web-based apps. Results are confidential and used as part of the hiring decision process.

$110,000 – $125,000 per year. Starting pay is commensurate with relevant experience. 21
