Position Title:

Registered Nurse, Non-Acute

Department/Unit:
Registered Nurse, Fairfield Primary Care

Shift/

Hours:

24 hours/week. Rotating schedule (possible one 12 hour shift and one 8-hour shift - two eight-hour shifts and one four-hour shift, etc.). It will change based on clinic needs, vacation coverage, flexibility with scheduling. Hour of Operation: M-Thurs 8am - 8pm, Friday 8am - 4:30pm, Sat 9am - 12:30pm, Closed on Sun & Holidays

Salary determined based on years of nursing experience.

The Fairfield Primary Care Clinic offers a variety of primary care services:
Same-Day Care of Illness, Well-Child Visits, Vaccinations and Immunizations, School and Sports Physical, Care and Treatment of Minor Injuries, Hearing and Vision Screening

Ideal Candidate: 2 years nursing, flexible with scheduling, strong communication skills for phone triage, competent in various primary care services

Key Responsibilities:

• Provide direct patient care, including administering immunizations, conducting screenings, and supporting well-child visits and sick visits

• Perform phone triage, assessing patient symptoms and providing clinical guidance and follow-up

• Support care coordination, including referrals, medication refills, follow-up appointments, and communication with external providers

• Educate patients and families on treatment plans, health maintenance, and chronic disease management

• Collaborate closely with physicians, nurse practitioners, medical assistants, and other interdisciplinary team members to ensure quality care

• Document nursing care accurately and timely in the electronic health record (EHR)

• Participate in quality improvement initiatives, staff meetings, and training opportunities

Qualifications:

• Associate's or Bachelor's degree in Nursing (BSN preferred)

• Current RN license in the state of Ohio

• Minimum of 2 year of nursing experience, preferably in pediatrics or ambulatory care

• BLS certification required

• Excellent communication, organizational, and critical-thinking skills

• Passion for working with children and their families in a collaborative care environment
