Job Description

The Hospice Registered Nurse is an experienced registered professional nurse who initiates and coordinates the hospice care plan, organizes, and directs hospice care, and is experienced in nursing with an emphasis on community health education. The professional nurse builds from community resources to plan and direct services to meet the needs of individuals and families within their homes and communities.
EssentialJob Responsibilities
The RN provides care to patients diagnosed with a terminal illness, reviews and updates the plan of care, performs prescribed medical treatments including pain management and symptom control, conducts assessments and evaluations, and provides education and supportive care to patient and family.
- Complete initial, comprehensive, and ongoing assessments of patient and family to determine hospice needs, providing a complete physical assessment and history of current and previous illnesses.
- Provide professional nursing care utilizing all elements of the nursing process.
- Assess and evaluate patient’s status by writing and initiating the plan of care, regularly reassessing needs, and revising the plan as necessary.
- Use health assessment data to determine nursing diagnoses.
- Develop a care plan that establishes goals based on nursing diagnoses and incorporates palliative nursing actions; include the patient and family in the planning process.
- Initiate appropriate preventive and rehabilitative nursing procedures; administer medications and treatments as prescribed in the physician’s plan of care.
- Counsel the patient and family in meeting nursing and related needs.
- Provide health care instructions to the patient as appropriate per assessment and plan.
- Assist the patient with activities of daily living and facilitate the patient’s efforts toward self‑sufficiency and optional comfort care.
- Act as Case Manager when assigned by Clinical Supervisor/Nursing Supervisor, coordinating patient care for the assigned caseload.
- Complete, maintain, and submit accurate clinical notes regarding patient condition and care provided; record pain/symptom management changes and outcomes.
- Communicate with the physician regarding patient needs, report changes in condition, and obtain/receive physicians’ orders as required.
- Communicate with community health personnel to coordinate the care plan.
- Teach the patient and family/caregiver self‑care techniques as appropriate; provide medication, diet, and other instructions ordered by the physician and recognize opportunities for health counseling.
- Work in concert with the interdisciplinary group to maintain a safe environment for the patient.
- Assist the patient, family/caregiver, and other team members in providing continuity of care.
- Work in cooperation with the family/caregiver and hospice interdisciplinary group members to meet emotional needs.
- Attend interdisciplinary group meetings.
- Registered Nurse with a valid CA state license.
- Hospice experience preferred; at least one year of recent hospice experience with a strong foundation of acute care experience.
- Maintain a current CPR certification.
- Valid driver’s license, auto insurance, reliable transportation, and willingness to drive to patient locations.
- Ideal candidates must possess excellent interpersonal skills, motivation, and a passion for hospice care.
