Weekend Admission Nurse; RN

Part time Registered Nurse Case Manager

Anticipated Territory:
Solano County Area. All opportunities at 21

HHS require being in the field visiting patients in their homes.

Schedule:

Must be available full days every Saturday and Sunday (4 SOCs/day). Also willing to consider full time:
Saturday and Sunday plus 3 weekdays.

• Conduct Start of Care (SOC) Admissions – OASIS
• Conduct follow‑ups, evaluation visits as needed in designated territory
• Make home visits to patients in designated territory
• Perform assigned duties, including medication reconciliation, wound care, treatments, and procedures
• Monitor clients’ conditions, reporting changes to the Clinical team
• Collaborate with physicians and agency staff to deliver the best care possible
• Initiate and administer IV medication infusions safely and effectively in accordance with established protocols
• Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided

• Current and unrestricted CA RN license
• Current BLS/CPR certification
• Valid CA driver’s license
• Experience conducting assessments and SOCs – OASIS highly desired
• High level of clinical knowledge in home health care preferred
• Experience with high acuity patients is a plus
• Experience working with an interdisciplinary team desired
• Case Management fundamentals desired
• Previous experience with IV medication infusions highly preferred
• Proficiency initiating intravenous therapy is a plus
