Currently hiring for Home Health RNs for Solano County, CA (opportunity to earn $90k - $140k yearly) with multiple openings.

Prohealth Home Care is seeking an empathetic Home Health Registered Nurse (RN) who will aim to make a positive difference in our community. Our focus is to provide skilled and dignified care in the comforts of their home while providing emotional support to our patients and their families.

- Sign-on bonuses of $10,000 for full-time RN's
- Provide admission, evaluation and follow-up skilled nursing visits for home health patients.
- Provide quality hands‑on care, management, and teaching of patient’s care plan in accordance with physician orders, under Clinical Manager's supervision.
- Coordinate appropriate care involving all other healthcare team members (such as Physical Therapists, Occupational Therapists, Home Health Aides, and Medical Social Workers).
- Report patient's progress to physician and Clinical Manager as required to maintain continuity of care.
- Implement plan of care with patient and family to assist them in achieving optimal outcomes.
- Valid RN License to practice in CA.
- Current BLS/CPR certification.
- Valid CA Driver’s license.
- Minimum one year nursing experience preferred.
- Med Surg, ICU, ER, Acute Experience, Home Health experience is a plus.
