Currently hiring for Home Health RNs for Solano County, CA (opportunity to earn $90k - $140k yearly) with multiple openings.

Prohealth is a locally owned Home Health, Hospice & Palliative Care agency with 4 locations serving over 12 counties in Northern California. We maintain an average daily census of over 1800 patients. • Provide admission, evaluation and follow-up skilled nursing visits for home health patients.
• Provide quality hands‑on care, management, and teaching of patient’s care plan in accordance with physician orders, under Clinical Manager's supervision.
• Coordinate appropriate care involving all other healthcare team members (such as Physical Therapists, Occupational Therapists, Home Health Aides, and Medical Social Workers).
• Report patient's progress to physician and Clinical Manager as required to maintain continuity of care.
• Implement plan of care with patient and family to assist them in achieving optimal outcomes.

• Valid RN License to practice in CA.
• Current BLS/CPR certification.
• Valid CA Driver’s license.
• Minimum one year nursing experience preferred.
• Med Surg, ICU, ER, Acute Experience, Home Health experience is a plus.
