Their health system includes two hospitals, a 100-provider primary and specialty care medical group, a cancer center; and a new medical fitness center.
Job Description:- The RN Case Manager will coordinate inpatient care, collaborate with multidisciplinary teams, ensure appropriate utilization of resources, facilitate discharge planning, and help achieve optimal patient outcomes across the continuum of care.
- BSN required
- Minimum 2 years of acute care nursing experience
- Minimum 2 years of Case Management experience
- Cerner & Epic experience preferred
- CCM or ACM certification preferred
- ED experience is a strong plus
