About the roleWe are searching for a Senior Case Manager to assist individuals in gaining access to medical, psychiatric, social, educational and other services. The primary goal of this role is to optimize the functioning of individuals who have complex behavioral health needs by coordinating the provision of quality treatment and support services. The Senior Case Manager will help persons optimize natural systems of support to their greatest advantage, promote recovery and develop resiliency.
Whatyou’ll do Intake and Assessment
- Screen prospective program participants for eligibility. Provide complete information regarding program services, expectations, rights and responsibilities.
- Obtain and review necessary case acceptance authorizations/consents including income verification and other documentation to determine eligibility for program and services; review with supervisor.
- Assess for risk and protective factors and respond to high risk factors including suicidal and homicidal ideation.
- Recognize and manage transfers/hand-offs and other critical points in treatment (critical risk points include but are not limited to: initial contact; change or transfer of care; change in legal status; change in life events; change in mental status; change in physical condition; change to a less restrictive level of care; discharge from services)
- Complete assessments including HMIS Intake, psychosocial assessments and housing retention assessment and use information to evaluate participant strengths, needs, and risk indicators.
- Maintain a caseload of up to 20-40 (depending on program design and requirements) individuals diagnosed with serious mental illness (SMI) in need of housing assistance, medical and psychiatric support.
- Conduct interviews, access resources to gather information for factual base on which to formulate a personalized, client-centered treatment plan.
- Demonstrate knowledge and understanding of participant health and social presenting risks and apply appropriate risk stratification.
- Establish participant goals based on appropriate resource utilization, participant consensus, and level of care and services required.
- Communicate participant care plan with support staff and supervisor during weekly check-ins.
- Provide timely completion of all case planning and implementation activities.
- Apply effective follow-up dates to care plan interventions.
- Perform routine comprehensive, independent reassessment of participants physical and mental health needs, abstinence from substance use, effectiveness of current/past services & interventions and progress toward achievement of plan goals. Make care plan adjustments in writing as appropriate.
- Utilize existing community resources and contracted partners in providing care/services to meet goals.
- Follow agency procedures to link with other SHELTER, Inc. services and staff supports.
- Maintain accurate, up-to-date documentation in the established case management record system.
- Act in accordance with laws and procedures governing confidentiality, release of information, consent, domestic violence reporting, mandated reporting, etc.
